Person Centered Planning: Qualified staff will assist our clients with developing, implementing, and reviewing their specific plan, so that it fits the needs of the client. This can include:
- providing a group home placement
- providing daily living supports to clients living on their own
- assisting in locating, developing and accessing community based residential opportunities
- acting as a liaison between community and other services
- providing and coordinating respite service
- provision of crisis assistance
2) Specific Programs:
Residential Supports: QCRS Inc. operates five group homes in the Charlottetown area, and one small option home in Stratford. Each home is staffed and operated on a 24 hour basis and provides long term residential support to clients who require attendant care, life skill development and assistance in daily activities.
Community Supports Program: QCRS Inc. offers our clients a variety of day opportunities, with individualized planning to help adapt to their needs. The planning of the clients' programs are based on their choices, abilities, age, medical and physical needs, as well as available resources.
